CLIENT CARE COORDINATOR
Home Instead – Beaverton Office

 

POSITION OVERVIEW
-The client care coordinator serves as a primary contact for our senior clients and their family members at all stages of the service relationship.
-Key job functions include conducting pre-service assessments, developing and implementing service plans, managing safety and risk in clients’ homes, and performing regular in-home quality assurance visits.
-You must be a licensed driver with current insurance as the job involves many home visits with our clients throughout Beaverton, Tigard, Tualatin, and Sherwood. On-the-job mileage is reimbursed.
-The client care coordinator reports to the client services supervisor and the agency director. 

EXPERIENCE, SKILLS, and TRAITS
-Qualifications: Common degrees, certifications, and credentials for this position are Certified Medical Assistant, Certified Nursing Assistant, Bachelor's or Master's degree in Social Work or Healthcare Management.
-Work Experience: A minimum of 3 years of solid work experience in health care social services is preferred. You must have an affinity for the elderly and good experience working with this population.
-Persuasive Skills: Pre-service assessments are your opportunities to engage families, determine their needs, and present our service as valuable solution. At least 80% of your pre-service consultations should result in new client service.
-Attributes for Success: Organization, process orientation, attention to detail and follow up, time management, systems and technology fluency, critical thinking, and a strong working memory
-Communication Expert: Professional and sensitive use of both written and spoken language are keys to effectively coordinating with office staff, CAREGivers, clients, and client representatives.
-Growth Mindset: Lifelong learner with the humility to receive and the confidence to give constructive feedback. Sustained pursuit of personal and professional growth. 

POSITION OBJECTIVES:
-Sign on new clients by assessing their needs and presenting our services as the best option to meet those needs. 
-Keep clients and their representatives happy through regular quality assurance visits in their homes and vigilant attention to detail and follow up. 
-Support CAREGivers to successfully and safely provide care in client homes.
-Maintain focus on the performance metrics of your role. Embrace the reality that this is a people and relationship business that is also driven by numbers.

SCHEDULE, COMPENSATION, AND BENEFITS:
-We are looking to make this a Tuesday - Saturday work week. (Sunday and Monday off)
-This position will be a 40 hour per week opportunity (8am to 5pm, with 1-hour lunch).
-Paid on an annual salary basis. Equivalent to $22 to $25 per hour. Open to discussion and negotiation.
-$3,000 to $4,000 available per year in performance bonuses.
