The client care coordinator serves as a primary contact for our senior clients and their family members at all stages of the service relationship. The client care coordinator reports directly to the client services manager and offers critical insights to improve our service delivery.
Some of the functions are as follows:
-Conducting pre-service assessments
-Developing and implementing service plans
-Managing safety and risk in clients’ homes
-Performing regular in-home quality assurance visits
You must be a licensed driver with current insurance as the job involves many home visits with our clients throughout Beaverton, Tigard, Tualatin, and Sherwood. On-the-job mileage is reimbursed.
EXPERIENCE, SKILLS, and TRAITS
-Social Work Background: The MSW credential is preferred and will result in a higher hourly wage. This position carries a client case load of up to 60 or 70 clients. Must be able to show empathy and care while working efficiently with many clients and situations each day.
-Communication Skills: A strong ability with written and spoken language are keys to effectively coordinating with office staff, CAREGivers, clients, and client representatives.
-Persuasive Skills: Pre-service assessments are opportunities to engage families, determine their needs, and if appropriate, influence them toward the solutions we provide. A high percentage of your pre-service consultations should result in new client service.
SCHEDULE, COMPENSATION, AND BENEFITS:
-This full-time position of 40 hours per week (8am to 5pm, with 1-hour lunch).
-$18 to $23 per hour (or higher) based on experience, credentials, and fit.
